CHANGES FOR THE 2020 – 2021 MCISD YEARLY PHYSICAL REQUIREMENTS
For the 2020 -2021 school year ONLY, you will not need a new physical exam if you are a current student-athlete with a current physical exam on file with your Mission CISD Athletic Trainer.
The following student-athletes will be required to complete a physical exam with their physician of choice:
1) ALL incoming 7th grade student-athletes, who do not have a 2019-2020 physical on file
2) All students who are not a current 2019 - 2020 student-athlete with a current physical exam on file with their Mission CISD Athletic Trainer

The rest of the REQUIRED forms can be filled out online at missoincisd.rankonesport.com
The MCISD Athletic Trainers will be notified if you answer "Yes" to any of the questions on the UIL Medical History. Any "Yes" answers to questions on the UIL Medical History form will require further medical review by the MCISD Athletic Trainers and also may require a physical examination & written clearance from a physician, physician assistant, chiropractor, or nurse practitioner.
If you are required to complete a physical exam for 2020 – 2021 school, the UIL has updated their forms. Only UIL Physical forms with 2020 printed on the upper Right hand corner of the UIL Medical History will be accepted (back side of the physical exam). This form may be found attached below or within Rank One online in the Mission CISD Forms in the upper Right hand corner in the Download and Print tab.
Mission CISD is utilizing an online version of the MCISD / UIL Athletic Signature forms in conjunction with Rank One Sport. Please complete the online version of the Mission CISD / UIL Athletic Signature forms by following the Red link below.

If you are required to take a physical exam for the 2020 - 2021 school year, the Pre-Participation Exam (PPE) and Medical History (MH) may be filled in online by the parent / guardian & the student-athlete, printed, and then taken the Medical Doctor (MD) for completion. Please make sure the student-athlete fills in their School Name and their School ID # and make sure the MD includes their printed name, address, date, and their signature. Also do not forget the signatures at the bottom of the MH. Please turn in your completed PPE & MH paper copy to the RCJH Athletic Trainer Anthony Saenz or your RCJH Coach - ONLY if you are required to complete a new physical examination.
The 2020 - 2021 MCISD Physical packet will expire on July 31, 2021 for MCISD Athletic purposes only.
All RCJH student-athletes must have all UIL / MCISD online forms completed on Rank One Sport prior to any participation in any practice, scrimmage, performance, or contest before, during or after school. This includes MCISD athletic periods.
